PLEASE make reservations as early as your plans permit as spots are limited. Reservations must be paid in full at the time of booking. Once payment is received your service will be confirmed.
ANY pickups after 12 pm will be charged a full day of services. (applied to boarding guest only)
CANCELLATION POLICY
ANY CLIENT WHO CANCLE SAME DAY OF ANY SERVICE WILL BE CHARGE 50% OF SAID SERVICE
ANY CLIENTS WHO CANCEL ANY SERVICES WE PROVIDE WILL ONLY RECIEVE CREDIT TOWARDS FUTRUE VISITS NO REFUNDS.
SERVICE AGREEMENT
I hereby acknowledge that CherishedPaws boarding and pet sitting, in consideration for our dog or dogs participating in overnight, and I hereby agree to all policies, procedures, costs, and any and all terms and conditions. This agreement shall be enforced from the date of signing and apply to each pet for all visits thereafter.
I agree to provide CherishedPaws with copies of vaccination records upon request, prior to any visit.
The pup is at least four months old, is in general good health, free of ticks and fleas, and on flea and tick preventatives.
Dog has never harmed another dog or any individual or displayed any sign of aggression towards others or individuals and has not harmed or shown any aggression or intimidation behaviors towards any person or animal.
Pup meets all municipal and state license and other requirements.
Dog has a temperament conducive for social interaction with other dogs to be boarded overnight. I understand due to the interactive nature, my dog may get dirty or smelly upon check-out.
Dogs are placed in groups with other dogs of the same energy level and size, and closely monitored. This may result in injuries, including but not limited to broken nails, sore paws, abrasions, lacerations, punctures, and other wounds and cuts, and be transferred and communicable illness, including but not limited to Bordetella virus and canine influenza.
HOUSE SITTING AGREEMENT
If you plan on house sitting, it's important to understand the following terms and conditions:
Access to the Premises:
1 The Homeowner Agrees to:
Provide the house sitter with seven days notice of any changes to the agreed first day of the house-sitting assignment.
Provide the house sitter with seven days notice of an inspection of the premises.
Ensure that the premises will be vacant and in a habitable condition for the house sitter to move in on the agreed first day of the house-sitting assignment.
2 The House Sitter Agrees to:
Be responsible for any act by any person the house sitter allows on the premises who breaks any terms of the agreement.
Not to sublet the premises without the homeowner's prior written permission.
Not to allow any person not listed in this agreement to live at the premises without the homeowner's written permission.
Care of the Premises:
1 The Homeowner Agrees to:
Ensure that the premises are reasonably clean and fit to live in.
Provide the house sitter or the homeowner's nominated contact person with a set of spare keys for the premises.
2 The House Sitter Agrees to:
Not to use the premises, or allow the premises to be used, for any illegal purpose.
Not to cause a nuisance or allow a nuisance to be caused on the premises.
Not to interfere, or allow others to interfere with the reasonable peace, comfort, or privacy of neighbors.
Not to keep any animal on the premises other than those listed in this agreement without the permission of the homeowner.
Not to block any sink or drain.
Not to cause or allow any damage to be caused to the premises.
To care for every item in the premises.
To regularly clean the bathroom and kitchen.
To maintain the grounds and garden in the same condition as at the start of this agreement.
To put all household rubbish and recycling out for regular collection.
To report any infestation of the premises to the homeowner and seek advice for solutions to the problem.
VETERNARIAN AUTHORIZATION
Authorization for CherishedPaws to Communicate with Veterinarian and Provide Medical Care to your Dog
(A) To ensure that your dog receives the best possible care, I, as the owner, authorize CherishedPaws to speak with my veterinarian and their staff. CherishedPaws may confirm my dog's vaccination status, date of surgical alteration, last day of visit, and medical history.
in the event of an emergency, CherishedPaws will make reasonable efforts to contact me for instructions on how to proceed with medical care or seek medical attention. CherishedPaws will also treat your dog at your direction as the owner and with the utmost duty to their animal, unless the dog requires immediate medical care.
Your dog will remain with CherishedPaws until you are able to pick them up.
B. In the event, CherishedPaws is unable to contact me. The veterinarian will rely on section A above as my consent to treat my dog, I hereby grant the veterinarian and CherishedPaws , the right to make matters necessary treatment decisions for my dog in the event the veterinarian is unavailable. CherishedPaws has the right to seek an alternative veterinarian to solve discretion.
C. If a dog passes away at CherishedPaws The owner will be contacted immediately if owner or emergency contact can't be reached within one hour CherishedPaws Will reserve the right to bring the Dog to a location permitted by law I understand I will fully be responsible for all costs related to any medical treatment, including without limitation, the cost of transportation and any cost incurred. I agree to indemnify and hold, harmless,
Cherishedpaws in any event of illness or death of the dog.
ACCEPTING AND ACKNOWLEDGING RISK
I fully understand that (A)there are risk involved with interactions between humans and dogs, as well as between dogs and other dogs, which may result in property damage or bodily injury, including but not limited to permanent, disability, sickness, or death to humans or dogs (B) there may be other risk of sickness, includes any illness, limited to Bordetella “kennel, cough“ canine Influenza , bloat, or any other form of contagious diseases. I fully accept and assume all risk and responsibility for all risks. I acknowledge, receiving a copy of this document, acknowledging that I have read it & understand it this agreement and release of liability In any relation for as long as I use CherishedPaws for any services by signing this agreement, I certify the accuracy of all the information provided about my pets for the more certifying that I have read and fully understand clauses expressed in agreement.
FEES
I acknowledge that all services and products provided by CherishedPaws must be paid for at the time of drop-off, either by credit card, cash, or check. By providing my credit card information, I authorize CherishedPaws to charge the card for any outstanding services or products that remain unpaid beyond their scheduled completion date. Additionally, I accept responsibility for any collection fees or costs associated with returned or declined debit charges.
SUPPLIES
Expectations for Pet Owners
During your pet's stay at CherishedPaws, we require that owners provide their pet's food and any necessary medication. Additionally, please provide a blanket for your pet's overnight stay.
ABANDOMENT OF PET
If Client does not pick up the Pet(s) on the date it is due to be picked up, CherishedPaws will exercise all of its rights under New Jersey law with regard to the abandonment of animals set forth below and Client agrees to pay for all costs related thereto, including attorney fees and court costs.
New Jersey Law Title 45:16-15. Humane disposal of unretrieved animals; notice to owner.
A veterinarian or boarding kennel may elect the humane disposal of an unretrieved animal no sooner than 4 days after the owner or agent thereof has signed or refused to sign for the receipt of a certified letter from the veterinarian or boarding kennel owner indicating intent to humanely dispose of the animal, which letter shall be sent to the owner's or agent's last known address. In the event the owner cannot be notified by certified mail, return receipt requested, the veterinarian or boarding kennel owner may elect humane disposal any time after 4 days following return of the certified mail receipt. The veterinarian or boarding kennel manager shall keep an accurate record of the date and method of disposal and the name, address and telephone number of the person or shelter receiving the animal. L.1979, c. 354, s. 3, eff. Jan. 29, 1980.
45:16-16. Financial liability of owner for treatment of animal
The humane disposal of an unretrieved animal shall not relieve the owner or agent thereof of any financial obligation incurred for treatment, boarding or care by the veterinarian or boarding kennel.
IN WITNESS WHEREOF, the parties hereto have executed this Agreement as of the date first above written, and each person, by signing his name below, agrees to be bound by each provision including any schedule referenced herein.
TERMINATION
A.Either CherishedPaws or Client may terminate this Agreement at any time by providing notice to the other. Upon termination, Client will immediately pick up Pet(s) and pay all charges that are due.
B. CherishedPaws also may terminate this Agreement at any time, if in its sole discretion, determines that the Pet(s) poses a danger to the health and safety of any person and/or animal. In the event of an emergency, if CherishedPaws is unable to care for the Pet(s) and the Client cannot be reached, Client hereby authorizes CherishedPaws to transfer the Pet(s) to a friend or relative and agrees to pay for all the charges related to such transfer.
